• **In order to view these forms, please update your Adobe Acrobat Reader and please use Internet Explorer**
• Please use the Fundraiser Revenue Potential Form  if your club or organization is hosting a fundraiser.  This form must be submitted two weeks in advance of the planned fundraising activity.
• Once completed, please print out your form, sign it  and turn the form in to Justin Montoya in the Administration Office for approval and calendaring
• Use of Facilities Requests are for events occurring after 3:00 PM, events hosted by Boosters or other outside organizations, events on weekends or holidays/non-school days, and events occurring in the Auditorium or the FUHS Stadium.
